A migraine diary can be as simple as buying a notebook and simply jotting down information whenever you have a headache. I don’t know about you, but writing this kind of information while I have a migraine is the last thing I want to do. And after the migraine is gone, it’s the last thing I want to remember! Yet, one of the first important steps in living with migraines, is to understand what kinds of things set them off.

To make it easier for you to keep a migraine diary, I have developed a page which you can circle or check most of your answers. I have also put together a page of common migraine triggers which you can look over and then write in the appropriate place on your migraine diary page. Use the back of the page if you need to jot down more detailed information. Share this information with your doctor who can then help you formulate an appropriate treatment plan.

1. Purchase a notebook.

2. Click on the following pages below and print out

3. Assemble pages in notebook.
